Idaho Childrens Health Insurance Program (SCHIP) Application Information

The Children's Health Insurance Program (SCHIP) provides health coverage to your children if they don't have health insurance for them already. If your child meets criteria for SCHIP, he or she can receive full-medical coverage including well-child visits, prescription drugs, hospitalization and various other services.

SCHIP provides health coverage for children where family income is outside Medicaid limits. Families qualifying to receive medical coverage for their children under the SCHIP may have to pay a monthly premium depending on their income. The monthly premium can be between $10 and $15 for each child receiving health insurance through SCHIP program.

SCHIP is a comprehensive health insurance plan for eligible children living in the state of Idaho. Children under the age of 19 years and not having any other medical insurance will qualify if their family income is within the limits of SCHIP.

Children enrolled for the program will have access to services such as doctor visits, immunization, dental and vision care, prescriptions etc.

If you have an uninsured child under 19 years in your household and your monthly income is within the limits defined by the program, you can apply for SCHIP. You will have to visit your local Health and Welfare office to apply. You can also request application form through phone.

You will be required to provide proof of identity, citizenship, as well as any other insurance your child had in the past along with your application. You will also be required to prove your income as stated in the application form.

Eligibility Requirements

In order to qualify, your child must be:

  • A U.S citizen or a legal alien
  • Resident of Idaho
  • Under the age of 19 years
  • Child should not have a private medical insurance
  • Family income is high for the purpose of Medicaid but it should be within SCHIP limits. Apply in Person

You can apply for SCHIP in person by visiting your local Health and Welfare office. Once you are at the office, you will be helped with rest of the things. You can call Idaho CareLine and ask for an application to be mailed to you. Call at 1 800 926 2588 or 2 1 1 to request application. You can either return the completed application by mail or drop it in person at your nearest Health and Welfare office.

What you need to know before you apply

When you apply for SCHIP, you will be required to provide a proof of citizenship along with identity and social security number. You will also have to provide residency documents and information about any health insurance your child had in the past. You will be required to provide a proof of your income and income tax payments.

You will have to submit all these documents with the application form.

If you are receiving benefits for your child under SCHIP in Idaho and your circumstances (financial and non-financial) have changed, you're responsible to report them to related office at the earliest. These changes may include change in income, change of phone number, change in social security number, or even a newly diagnosed disability.
